Application Process

To be considered for enrollment, all prospective students, PreSchool - 5th Grade, need to:

  1. Complete the online application,
  2. Upload the documents listed below:
    • State Issued Birth Certificate
    • Updated Immunization Record *
    • Most Recent Report Card, Grades 1-5th
    • Educational/Diagnostic/Evaluative Testing Results/IEP/504, if applicable
    • Custodial Documentation, if applicable
  3. Pay the non-refundable $50 application fee:

* In accordance with Diocesan Policy, schools in the Catholic Diocese of Salt Lake City do not accept personal or religious immunization exemptions.  A medical exemption must be obtained from the student's pediatrician and must be on the practice’s letterhead. The student’s pediatrician must be licensed to practice in the State of Utah and maintain a medical practice in the State of Utah.


Application Information

Assessment

All applicants, Grades K-5, need to complete an educational assessment. An assessment will be administered in January, February and March. Applicants will be notified to register for an assessment date.

  • Grades K-2: Written & Oral Assessments
  • Grades 3-5: Star Math & Reading Assessments & Writing Prompt

Current Year Admissions

Admission Timeline
2025-2026 School Year

  • Priority Sibling Admission: Beginning September 16, 2024, siblings will be accepted on a first come first serve basis.
  • Prospective Student Early Admission: September 16, 2024 – January 10, 2025 Acceptances will be sent by February 7, 2025.
  • Prospective Student Rolling Admission: January 11, 2025, through the 2025-2026 School Year. Acceptances will be sent after February 7, 2025, on a space available basis.

Acceptance Guidelines

  • Siblings of current students
  • Students whose families are registered at the host parish (if applicable)
  • Students from feeder parishes
  • Non-Catholic students

Transfer Students

It is the expectation of the Catholic Schools Office that administrators communicate when a student from another Utah Catholic School applies for admission.  This communication should take place prior to the applicant completing the educational assessment and being accepted.
